چکیده

L ower limb muscle atrophy and weakness are common in patients with chronic obstructive pulmonary disease (COPD) and make an important contribution to limiting their exercise tolerance [1]. Muscle disuse, hypoxaemia, malnutrition, oxidative stress and systemic inflammation have all been portrayed as possible causes of muscle dysfunction in COPD [2]. As far as systemic inflammation is concerned, the available literature supports cytokine-mediated effects on peripheral muscle atrophy, dysfunction and regeneration [3].

Mechanisms of Physical Activity Limitation in Chronic Lung Diseases

In chronic lung diseases physical activity limitation is multifactorial involving respiratory, hemodynamic, and peripheral muscle abnormalities. The mechanisms of limitation discussed in this paper relate to (i) the imbalance between ventilatory capacity and demand, (ii) the imbalance between energy demand and supply to working respiratory and peripheral muscles, and (iii) the factors that indu...

Quadriceps wasting and physical inactivity in patients with COPD.

Quadriceps weakness is an important complication of advanced chronic obstructive pulmonary disease (COPD) but few data exist concerning muscle bulk in early disease. We hypothesised that quadriceps bulk, measured by ultrasound rectus femoris cross-sectional area (USRF(CSA)), would be reduced in mild, as well as advanced, COPD compared with controls, and would correlate with physical activity. 1...

Skeletal muscle weakness, exercise tolerance and physical activity in adults with cystic fibrosis.

The aim of the present study was to investigate the prevalence of muscle weakness and the importance of physical inactivity in cystic fibrosis (CF), and its relationship to exercise tolerance and muscle strength. Exercise tolerance, skeletal and respiratory muscle strength were studied in a group of 64 adults with CF (age 26+/-8 yrs, FEV(1 % predicted) 65+/-19) and in 20 age-matched controls. P...

Impaired arm activity in COPD: a questionable goal for rehabilitation.

The most common extrapulmonary problems in chronic obstructive pulmonary disease (COPD) patients are cardiovascular comorbidities, muscle weakness and dysfunction [1–3]. Indeed, the reduced peripheral muscle mass and strength increase patient’s symptoms during effort (fatigue and or dyspnoea) and progressively lead to disability. Furthermore, both lower and upper limb dysfunction has been shown...

Skeletal muscle dysfunction: a ubiquitous outcome in chronic disease?

Skeletal muscle wasting is a debilitating consequence of respiratory disease, particularly well characterised in individuals with chronic obstructive pulmonary disease (COPD). However, it remains controversial whether peripheral muscle dysfunction is an intrinsic part of the respiratory disease process, or a generalised response to multiple atrophy stimuli. Such debate has not only informed the...
